Enroll In Services

To enroll in any of Center Point DAAC's services you must complete our screening and assessment process. This process ensures that you receive the appropriate services based on your individual needs. Once your assessment is complete, you will complete the new client paperwork and a drug test prior to starting services.

You may start the enrollment process in one of three ways:

1. Call us at (707) 544-3295 between the hours of 9 AM and 9 PM Monday through Thursday. 9 AM to 5 PM on Friday. One of our Intake Counselors will assist you in starting the process.

2. Complete the form below. One of our Intake Counselors will process your information and reach out to schedule your screening and assessment.

3. Visit us in-person during Open Enrollment. Open Enrollment is every Tuesday from 10 AM to 7 PM at 2403 Professional Drive, Suite 101, Santa Rosa, CA 95403. Clients are enrolled in the order in which they arrive by one of our Intake Counselors. 
 

Most of our client services are covered by Medi-Cal, Medicare, or a County/State/Federal funded program. If you do not qualify for any of these programs, we do offer reasonable private-pay rates. Please call us for private-pay rate information. We are working to provide scholarships and accept private insurance in the future.
